If you're turning 65 in the Dallas–Fort Worth area, your Medicare clock has already started. The decisions you make in the months around your birthday affect your coverage, your costs, and whether you face lifelong penalties. The good news: once you understand the timeline, it's straightforward.
Your Initial Enrollment Period (IEP)
Your first chance to sign up is a seven-month window built around your 65th birthday:
- The three months before the month you turn 65
- The month you turn 65
- The three months after
Enrolling in the three months before your birthday month is usually best, because your coverage can begin the first day of your birthday month with no gap. Wait until after, and your coverage start date can be pushed back.
What you're actually signing up for
Turning 65 means deciding how to put your coverage together:
- Part A (hospital) — most people pay no premium and should enroll on time.
- Part B (medical) — has a monthly premium; delaying it without other qualifying coverage can trigger a lifelong penalty.
- Then you choose a path: Original Medicare (often with a Medicare Supplement plus a Part D drug plan), or a Medicare Advantage plan that bundles everything together.

If you're still working at 65
Many people in DFW keep working past 65. If you have qualifying employer coverage, you may be able to delay Part B without penalty and use a Special Enrollment Period later when you retire. But the rules depend on your employer's size and plan — it's worth confirming before you delay anything, because guessing wrong can be costly.
Watch the penalties
Two penalties catch people off guard:
- Part B late penalty: roughly 10% added to your premium for each full 12 months you were eligible but didn't enroll (and didn't have qualifying coverage) — for life.
- Part D late penalty: a smaller, ongoing surcharge if you go without creditable drug coverage.
Both are avoidable with the right timing.
